Getting Help Paying Medicare Costs in Virginia

Learn about Extra Help, Medicare Savings Programs and Medicaid, and where Virginians can apply or get counseling at no cost.

Medicare premiums, deductibles and drug costs can add up. If money is tight, there are programs that may help. Each has its own rules, and the limits change every year. If you are not sure you qualify, it is still worth asking.

Extra Help with drug costs

Extra Help is a program for people with limited income and resources. It helps lower or cover the costs of Medicare Part D prescription drug plans.

You can apply before or after you join a Part D plan. Ways to apply:

  • Online at SSA.gov
  • By phone with Social Security at 1-800-772-1213, Monday through Friday, 8 a.m. to 7 p.m. Help is offered in English, Spanish and other languages. If you are deaf or hard of hearing, call 1-800-325-0778.

It helps to have bank statements, tax returns and statements for pensions or other income ready.

Medicare Savings Programs

Medicare Savings Programs are run by each state through Medicaid. They help people whose income or resources are too high for full Medicaid. In Virginia:

  • The Qualified Medicare Beneficiary program pays Part A and Part B premiums, plus Part A and Part B cost-sharing, like deductibles and coinsurance.
  • The Specified Low-Income Medicare Beneficiary program and the Qualifying Individual program pay the Part B premium.

Good to know: everyone enrolled in a Medicare Savings Program is automatically enrolled in Extra Help too.

Medicaid for people with both

Some people qualify for both Medicare and full Medicaid. Medicare pays first for the services it covers. Medicaid pays last.

Depending on your coverage, Medicaid may help pay your Medicare costs. It may also pay for things Medicare usually doesn't, such as nursing home care and personal care services.

People with both Medicare and full Medicaid are automatically put in a Medicare drug plan and automatically get Extra Help.

Where to apply in Virginia

For Medicaid or a Medicare Savings Program in Virginia, you can apply:

  • Online at commonhelp.virginia.gov
  • By phone with Cover Virginia at 1-855-242-8282 (TDD, a telecommunications device for the deaf: 1-888-221-1590)
  • In person, by mail or by fax through your local Department of Social Services

Free counseling from VICAP

The Virginia Insurance Counseling and Assistance Program (VICAP) gives people with Medicare free, impartial and private help. VICAP counselors do not sell insurance. They can help with:

  • Low-income help programs like the ones on this page
  • Comparing Part D drug plans, Medicare Advantage plans and Medigap
  • Appeals and denials

VICAP counseling is offered through your local Area Agency on Aging. Call them and ask to meet with a VICAP counselor.
